# Nurses in Indonesia: duties, STR licence, pay and applying safely

## What does a nurse do in Indonesia?

Nurses care for patients directly: they assess patients, monitor vital signs, provide nursing care and record everything in the medical record. They work alongside doctors and other health workers in hospitals, community health centres (puskesmas), clinics or home care, usually on rotating shifts. Explaining care to patients and their families in plain language is a large part of the job.

## What do you need to become a nurse in Indonesia?

You need a nursing qualification, a pass in the national competency exam and an STR registration. Law 17/2023 Article 213 requires vocational and professional programme students to sit the competency exam, and Article 260 requires every practising health worker to hold an STR. The Health Council issues the STR on the Minister's behalf once the diploma or professional certificate and the competency certificate are in place, and it is valid for life. Some places of practice also require an SIP from the regency or city.

## What does a typical nurse job description look like?

Nurse vacancies in Indonesia usually ask for a D3 nursing diploma or a Ners qualification with an active STR, willingness to work rotating shifts, and good communication with patients. Some also ask for a basic life-support training certificate. We wrote the description below ourselves from common advertising patterns. ## How much do nurses earn in Indonesia per month?

We do not show a nurse-specific pay figure yet, in hospitals or community health centres, because no data meets our standard: verified vacancies with a large enough sample, licensed data or an official role-level pay scale. The table below shows only the data that actually exists. As a floor, Government Regulation 36/2021 says workers with less than one year of service may not be paid below the minimum wage where they work, which is the 2026 minimum wage (UMK) of the regency or city.

## How do you become a nurse in Indonesia?

The main routes are a D3 nursing diploma for vocational nurses, or a nursing degree followed by the Ners professional programme. Law 17/2023 Article 210 sets a three-year diploma as the minimum for health workers, so a vocational high school (SMK) health graduate cannot become a nurse without further study. Under Article 212, degree graduates may practise only after the professional programme. For work abroad, also plan language courses and tests.

## Can Indonesian nurses work abroad?

Yes, through official routes that differ by country. Japan's EPA programme accepts Indonesian nurse candidates with a nursing licence and two years of experience; they work while preparing for Japan's national nursing exam. Germany's Triple Win programme is run by the Federal Employment Agency and GIZ under an agreement with BP2MI, and the UK accepts nurses who join the NMC register and are sponsored by an approved employer. ## Which workplace rights should nurses check?

Law 17/2023 Article 273 gives health workers the right to fair pay, health and employment social security, and occupational safety protection. Employers must register their workers with BPJS (Law 24/2011 Article 15). Government Regulation 35/2021 requires overtime pay when you work beyond the applicable working hours. Ask for a written employment agreement that states the contract type, pay and shift pattern before you start.

**Legal status:** the Labour Protection Law was approved by the DPR and the Government on 6 October 2026 but has not been promulgated and its text is not public. Until it takes effect, the rules on this page apply.

## What does a nurse's career path look like?

Nurses usually start as staff nurses, then move up to nurse in charge or ward head as their experience and competence grow. Law 17/2023 Article 218 opens specialist education for health workers, including specialist ners. Nurses can also move into teaching, service management, home care, hajj health teams or hospital administration. The SIP is renewed every five years with proof of enough professional credit units.

## How do nurses reach Japan?

Two routes are often confused. The **EPA programme** (Economic Partnership Agreement) is a government-to-government scheme under the Indonesia-Japan economic partnership agreement, in force since 1 July 2008. According to Japan's Ministry of Health, Labour and Welfare (MHLW), Indonesian nurse candidates must hold an Indonesian nursing licence and have two years of work experience. In Japan, JICWELS is the only body that coordinates EPA placements, so a private recruiter selling an "EPA route" should be treated with suspicion.

EPA candidates work in a hospital on an employment contract while studying for Japan's national nursing exam, with pay at least equal to a Japanese worker and the protection of Japanese labour law. Since the amended agreement took effect in August 2026, the stay limit for Indonesian candidates is five years. Once you pass the exam, you can keep working as a nurse in Japan.

The second route is **SSW nursing care** (Specified Skilled Worker, kaigo). This is care work, not practice as a licensed nurse. According to the MHLW, it requires a care skills test and a care-specific Japanese test, in addition to the general Japanese test. The care tests are held in several Indonesian cities, including Jakarta, Surabaya, Bandung and Medan.

## How do nurses reach Germany?

The **Triple Win** programme has been run by Germany's Federal Employment Agency with GIZ since 2013, under an agreement with BP2MI. According to the official programme page, applicants are registered nurses with a nursing diploma, an active STR and at least one year of experience in a hospital or care institution. German is an advantage but not required when you apply.

Participants take German classes up to B1 level in Bandung, then start the recognition of their professional qualification after arriving in Germany. The first work visa is valid for one year and becomes a residence permit once the participant passes the recognition exam and a B2 German exam. Selection to departure takes about twelve to fourteen months. You apply through your own SISKOP2MI account.

## How do nurses reach the United Kingdom?

Nurses can work in the UK on the Health and Care Worker visa. According to GOV.UK, you need a confirmed job offer from a Home Office approved employer, a certificate of sponsorship and, for new applicants, English at B2 level. Before working as a nurse you must also join the Nursing and Midwifery Council (NMC) register, which tests overseas applicants with a computer-based test (CBT) and a clinical skills exam (OSCE), and asks for English evidence such as IELTS or OET.

Note that the care worker and senior care worker codes may only be used to extend or switch a visa inside the UK. A "UK care worker visa" offered to applicants still in Indonesia does not fit those rules.

## What about the Middle East?

For nurse vacancies in the Middle East, check that the placement agency (P3MI) and the vacancy are listed on SISKOP2MI before you hand over documents. In Saudi Arabia, the Saudi Commission for Health Specialties (SCFHS) classifies and registers health professionals, verifies certificates of practitioners educated abroad and runs licensing exams. Regulators abroad often ask for proof of your registration status; Indonesia's Health Council issues a Certificate of Good Standing confirming that the holder is registered and not under sanction.

Interview questions for nurse jobs often cover patient safety, dealing with anxious patients or families, keeping medical records confidential, setting priorities, and your professional registration.
