# Physiotherapists in Indonesia: duties, training, STR and SIP, pay and working abroad

## What is a physiotherapist in Indonesia, is it a doctor, and what do they do?

A physiotherapist is a health worker, not a doctor, who helps patients restore, maintain and develop movement and body function. Ministry of Health Regulation 65/2015 defines physiotherapy as care using manual handling, movement training, physical, electrotherapeutic and mechanical equipment, functional training and communication. Physiotherapists assess complaints, plan therapy, train patients, track progress and record it, for muscle and bone, nerve, heart and lung problems and child development.

- Give exercise therapy, manual therapy and treatment with physical or electrical equipment within their authority.
- Train patients after a stroke, surgery or sports injury to return to daily activity.

- Teach patients and families home exercises and safe ways to move.
- Make home visits under a facility's assignment or from a licensed independent practice.
- Record assessments, treatment and progress in the medical record.

## What do you need to become a physiotherapist in Indonesia?

You need a physiotherapy qualification, a pass in the competency exam and an STR. Law 17/2023 Article 213 requires vocational and professional students to sit the competency exam, Article 212(2) says degree graduates may practise only after the professional programme, and Article 260 requires the STR. Health workers who serve patients directly also need an SIP (elucidation of Article 263); Government Regulation 28/2024 Article 682 sets one SIP per place of practice for five years.

## What does a typical physiotherapist job description look like?

Physiotherapist vacancies in Indonesia usually ask for a D3 in physiotherapy or a professional physiotherapist qualification with an active STR, the ability to assess patients and plan exercise programmes, good communication with patients and families, and willingness to make home visits. Sports and child development clinics often ask for experience in those fields. ## How much does a physiotherapist earn in Indonesia?

KarirAman does not yet show a pay figure for physiotherapists because no data meets our standard. Ministry of Health Regulation 13/2025 Article 216 requires health workers' wages to be at least the minimum wage, Article 217 asks facilities to prefer employment over partnership arrangements, and Article 218 says partner health workers receive service fees under a cooperation agreement. For home visits, agree fees, the fee split and transport costs in writing before you start.

## Which education routes lead to physiotherapy in Indonesia?

There are two routes. The vocational route is a D3 in physiotherapy or a D4 applied bachelor's, ending with the competency exam. The professional route is a physiotherapy degree followed by the professional physiotherapist programme, because Law 17/2023 Article 212(2) says health degree graduates may practise only after that programme. A three-year diploma is the minimum for health workers (Article 210), so vocational school (SMK) graduates must study further first.

## Can Indonesian physiotherapists work in Japan, Germany or Australia?

Yes, but the destination country must first recognise your qualification. In Japan, the residence status for medical professionals requires a Japanese physical therapist qualification. In Germany, using the title Physiotherapeut needs a permit, including an equivalence check of your training and German language skills. In Australia, the Australian Physiotherapy Council assesses overseas-qualified physiotherapists before registration with Ahpra, which also has an English language standard. Details follow below.

## What should physiotherapists check before home visits?

A home visit needs a lawful basis. Ministry of Health Regulation 13/2025 Article 175 says health workers at a facility may practise in patients' homes when assigned by their facility, and those in independent practice may provide home care in the regency or city that issued their SIP. Ask for an assignment letter, log every visit and share your schedule with a colleague. Law 17/2023 Article 273 lets you stop care if you face violence or harassment.

## Which workplace rights should physiotherapists check?

Law 17/2023 Article 273 gives health workers the right to fair pay, occupational safety and health protection, health and employment social security, and protection from degrading treatment. Ministry of Health Regulation 13/2025, in force since 3 November 2025, requires the wage structure and scale to be set out in the employment contract (Article 219) and gives employers at most one year to adjust their pay rules (Article 307). Ask for a written contract before you start.

**Legal status:** the Labour Protection Law was approved by the DPR and the Government on 6 October 2026 but has not been promulgated and its text is not public. Until it takes effect, the rules on this page apply.

## How is a physiotherapist different from a spa or massage therapist?

A physiotherapist is a health worker who treats movement and function problems based on clinical assessment, with an STR and SIP. A spa therapist provides wellness treatments with massage, herbal preparations and aromatherapy; Ministry of Health Regulation 8/2014 on spa health services requires every spa therapist to hold an STPT registration from the regency or city health office. Spa massage is not physiotherapy, and Law 17/2023 Article 312 bars anyone from using a title or method that gives the impression of being a registered health worker without the right to.

For job seekers this means a "therapist" vacancy at a spa or massage parlour must not ask you to work as a physiotherapist if you do not hold an STR, and a physiotherapist working in a spa is still bound by professional standards. ## How do physiotherapists get to work in Japan?

Japan's Medical Services status of residence (医療) covers licensed health professionals. For applicants other than doctors and dentists, the Immigration Services Agency asks for a copy of a document proving one of the listed Japanese qualifications, which include 理学療法士 (physical therapist). In other words, you must first obtain a Japanese physical therapist qualification before you can work as a physiotherapist there.

## How do physiotherapists get to work in Germany?

In Germany, anyone who wants to use the title Physiotherapeutin or Physiotherapeut needs a permit (Erlaubnis) under section 1 of the Masseur- und Physiotherapeutengesetz (MPhG). Section 2 requires equivalent training, reliability, fitness for the job and the German language skills needed for the work. Training completed outside Germany is checked for equivalence; if it is not equivalent, applicants prove their knowledge through a knowledge test (Kenntnisprüfung) or an adaptation course of up to three years, and may choose between them. Treat any agent who promises physiotherapy work in Germany without this process with suspicion.

## How do physiotherapists get to work in Australia?

The Australian Physiotherapy Council describes itself as the assessing authority for overseas-qualified physiotherapists who want to practise in Australia: it assesses qualifications and skills for registration and migration purposes. After you pass the assessment and receive the final certificate, you still need general registration from Ahpra, Australia's health practitioner regulator. Ahpra says every applicant for registration, including overseas-qualified applicants, must meet their National Board's English language standard, with new minimum scores in effect from 23 April 2026. Prepare for the English test early.

## What should you check before accepting a physiotherapy job abroad?

1. Check the vacancy and the placement agency (P3MI) on SISKOP2MI. Ministry of Health Regulation 13/2025 Article 123 says health workers placed abroad as migrant workers go through licensed placement providers.
2. Remember the order of the rules: Law 18/2017 Article 30(1) says Indonesian migrant workers cannot be charged placement costs; Ministerial Regulation P2MI 17/2025 puts placement cost components on the employer and allows charging the worker only for costs written and itemised in the Placement Agreement; any request for money outside that document, in cash or to a personal account, is a red flag.
3. Make sure the offer clearly describes the qualification recognition and professional registration process in the destination country.
4. Keep your original diploma, STR and passport yourself.

**Does a physiotherapist's STR need renewing in Indonesia?**

No. Law 17/2023 Article 260(4) makes the STR valid for life unless it is suspended or revoked. What you renew is the SIP, which lasts five years for one place of practice and is renewed with the STR, a place of practice and enough professional credit units (Government Regulation 28/2024 Article 682). According to the KKI website, from 12 October 2026 registration runs only through SATUSEHAT SDMK.
