# Spa and massage therapists: duties, certification, STPT, pay, work abroad and red flags

Spa and massage therapists work in spas, hotels and on cruise ships and need an STPT. Beware 'spa plus' ads and unofficial spa jobs abroad.

- An ad promising big pay for 'plus' services is a sign of exploitation; in 2026 Lampung police investigated the recruitment of two children as 'plus-plus' therapists.
- Spa work abroad must use an official channel, a licensed P3MI or the individual route (Law 18/2017 Article 63); migrant workers cannot be charged placement costs (Article 30(1)).
- On 3 January 2025 the Constitutional Court held that spa services are part of traditional health services, not entertainment.

## What does a spa or massage therapist do?

Spa therapists give relaxation and wellness treatments: body massage, reflexology, body scrubs, masks and wraps, manual facials, and foot and hair treatments. Permenkes 8/2014 defines spa health services as holistic care combining water, massage, herbal preparations and aromatherapy. Therapists work in day spas, hotel or resort spas and wellness centres. The national competency standard for spa therapy (SKKNI SPA Terapi, Ministerial Decree 267/2023) sets out 84 competency units. To work, a spa therapist needs an STPT from the district or city health office (Permenkes 8/2014, Article 13).

- Greet clients, ask about their condition and complaints, then suggest a suitable treatment.
- Give body massage, reflexology or traditional massage to the workplace's standards.
- Do body scrubs, body masks and wraps, manual facials, and foot or hair treatments.
- Prepare treatment rooms, linen, essential oils and clean, sterile equipment.
- Spot contraindications such as fever, breathing problems or allergic reactions and report them to a supervisor.
- Record client visits and treatments as the workplace requires.
- Refuse client requests that conflict with service standards and norms.

## What do you need to become a spa therapist?

Permenkes 8/2014 Article 13 requires every spa therapist to hold an STPT from the district or city health office, and the application needs a competency certificate from a certification body. Ads usually ask for a senior high school diploma and massage training or experience. The same regulation gives therapists the right to refuse client requests that conflict with service standards and norms (Article 16), so an ad hinting at 'plus' services is a red flag, not therapist work.

## What does a typical spa therapist job description look like?

Spa therapist ads usually list the treatments you should know (traditional massage, reflexology, scrubs, facials), experience, certificates, shift pattern and spa location. Some ads ask for photos, an age limit, a gender or a certain appearance; we do not list such criteria as normal requirements. The description below is our own summary of common ads, not a vacancy from any spa, hotel or agency.

- A spa therapist competency certificate from a BNSP-licensed LSP, or willingness to take the competency test.
- An STPT from the district or city health office, or willingness to apply with the workplace's help.
- Experience in a day spa, hotel or resort spa, or wellness centre.
- For hotel spas or work abroad, basic English.
- Willing to work shifts, including weekends and public holidays.

## How much does a spa therapist earn?

KarirAman has no spa therapist pay data that meets our standard, so that row in the table reads 'not enough data'. Spa therapist ads often combine a base wage with a commission per treatment and tips, so income can go up and down. Ask for the base wage, how commission is calculated and the pay date in writing. The minimum-wage row in the table is the floor for businesses under PP 36/2021.

## Which training and certification routes lead to spa therapy?

Common routes are a vocational school (SMK) in beauty or hospitality, a private training institute (LPK) or a government training centre (BLK), followed by a competency test. The SKKNI SPA Terapi (Decree 267/2023) is the reference for testing, and Permenkes 8/2014 Article 12 grades spa therapists as pratama, madya and utama. On 8 October 2026 the BNSP directory listed actively licensed LSPs with spa therapist schemes. Check an LSP's licence before paying a test fee.

## Can spa therapists work abroad?

Yes, through official channels. In December 2025 the Ministry for Migrant Worker Protection sent wellness therapists to placements in the Maldives and Turkey, and in August 2026 it listed spa therapists among hospitality jobs for Turkiye, Malaysia, the Maldives and other countries. Law 18/2017 Article 30(1) says migrant workers cannot be charged placement costs; Regulation P2MI 17/2025 makes the employer bear placement costs by default, and you may be charged only costs written and itemised in the Placement Agreement; any other request for money is a red flag.

## Is baby spa part of a spa therapist's work?

Partly. The SKKNI SPA Terapi includes children's spa treatments and pregnancy massage, and the domestic-worker standard (SKKNI Pekerja Domestik, Decree 239/2022) includes baby massage, baby gym and accompanying babies in the pool. When the service is combined with health checks or care for mothers and babies, it belongs to health workers such as midwives and nurses, who need an STR and SIP under Law 17/2023. Know the limits of your role.

## How can you check that a spa is legitimate?

Spa businesses are now licensed through risk-based business licensing in the OSS system. PP 28/2025 Article 170 places spas under tourism-sector licensing, and Permenparekraf 4/2021 classes spa activity (KBLI 96122) as medium-high risk, which under PP 28/2025 Article 132 needs an NIB and a standard certificate. The spa standard asks for that certificate within one year of opening, a health certificate and therapists with an STPT. Full permits are no guarantee: in the 2016 Bali spa case, the owner could show permits and worker contracts.

## How do you apply for a spa therapist STPT?

Under Permenkes 8/2014 Article 13, a spa therapist applies for an STPT to the head of the district or city health office, individually or as a group through the workplace. The file includes a therapist biodata form, a copy of your ID card, a recommendation from a registered spa association, a copy of your therapist competency certificate from a competency certification body or LSP, a cover letter from the local puskesmas, passport photos and the technical permit of your current or planned workplace. The health office must decide within 14 working days of receiving the application.

This regulation predates Law 17/2023 on Health, so your local procedure may already have changed. The spa business standard in Permenparekraf 4/2021 writes the document as Surat Tanda Penyehat Tradisional, so the name on a local form may differ even though the abbreviation is still STPT. Ask the health office directly, and never pay a middleman who promises an STPT without a competency test.

## Where can you report being forced into 'plus' services or held at a workplace?

If you or someone you know is forced into sexual services, is not allowed to go home or has their documents held, it may be human trafficking under Law 21/2007. In an emergency, call the police. Women and children can report to SAPA 129. If it happens abroad, contact KP2MI or the nearest Indonesian mission. **How to answer:** Say you work to the written service procedure, refuse requests outside it and report them to your supervisor. Ask for a written job description before accepting the job.

- **Why did you apply to this company?** Mention one or two real things you learned from the company's official website, such as its business or services, and link them to your skills.
- **Why are you interested in this role?** Pick two tasks from the job advert, describe similar things you have done, and say which part you most want to grow in.
- **What are your strengths?** Name one or two strengths and give short proof: the situation, what you did and the result.
- **What is your weakness?** Choose a real weakness that is not central to the job, then explain what you are doing to improve it.
- **Tell me about a problem at work or school that you solved.** Use the STAR pattern: Situation, Task, Action, Result. Focus on what you yourself did and what you learned.
- **How do you work in a team, including when people disagree?** Give a real example of listening, explaining your view politely and finding a solution everyone accepted.
- **How do you handle pressure or tight deadlines?** Explain how you set priorities, such as a task list or checking with your supervisor, and give one example of the outcome.
- **Where do you see yourself in two or three years?** Mention the skills you want to master and the responsibility you hope to take on, without promising things you cannot know.
- **What salary do you expect?** First research the pay range for the role and the local minimum wage where you would work. Give a range, then ask whether the figure is gross or net and which allowances apply.
- **When can you start?** Give a realistic date. If you are still employed, mention the notice period in your current contract so you do not break it.
- **Do you have any questions for us?** Ask about the contract type, working hours, base pay and allowances, the next selection step and when results are announced. Ask for the offer in writing.

**Do spa therapists need an STPT?**

Yes. Permenkes 8/2014 Article 13(1) says every spa therapist must hold an STPT issued by the district or city health office. The application includes, among other things, a copy of your ID card, a recommendation from a spa association, a competency certificate from a certification body, a cover letter from the local community health centre (puskesmas) and the workplace's technical permit. Local procedures can differ, so ask your health office.

**Can a therapist refuse a client's request?**

Yes. Permenkes 8/2014 Article 16(d) gives spa service providers the right to refuse client wishes that conflict with spa service standards and prevailing norms. If a workplace forces you to provide sexual services or anything outside the standard, that is not therapist work. Leave the situation when it is safe and report it.
