Once it is issued, you apply for a national type D visa with work rights at the Polish Embassy in Jakarta through the e-Konsulat system. If an agency in Indonesia is involved, it must be an active P3MI on SISKOP2MI.

How can you check a job offer in Poland?
Ask for a copy of the work permit in your name and a written pre-employment contract in Polish and English stating pay, hours and duties. Refuse any 'work permit fee', because the employer pays the application fee and anyone demanding a benefit from a foreigner for a work permit is fined under Polish law. Any cost charged to you must be written and itemised in the Placement Agreement.
