What does a midwife do in Indonesia?
A midwife cares for women and babies before, during and after birth: antenatal check-ups, attending normal births, postnatal care for mother and newborn, and family planning services within their authority. Midwives also support immunisation and refer high-risk cases to a doctor or hospital. Law 17/2023 Article 199(4) divides the midwifery workforce into vocational midwives (bidan vokasi) and professional midwives (bidan profesi). To practise, a midwife must complete midwifery education and pass the competency exam, then hold an STR and SIP.

What do you need to become a midwife in Indonesia?
You need a midwifery qualification, a pass in the national competency exam and an STR registration. Law 17/2023 Article 213 requires vocational and professional students to sit the competency exam, and Article 260 requires every practising health worker to hold an STR. The Health Council issues the STR on the Minister's behalf, and it is valid for life. To practise, a midwife also needs an SIP from the regency or city, one SIP per place of practice.

Midwife vacancies in Indonesia usually ask for a D3 midwifery diploma or a professional midwife qualification with an active STR, willingness to work rotating shifts and on-call duty, and good communication with mothers and families. How much does a midwife earn in Indonesia?
For civil-service (PNS) midwives, the table below shows official base pay under Government Regulation 5/2024 for grades II/c to II/d and for grade III; a midwife's grade depends on appointment and promotion, and allowances are not included. For private-sector midwives, KarirAman has no data that meets our standard yet. As a floor, Government Regulation 36/2021 bars pay below the minimum wage for workers with less than one year of service.

Which education routes lead to midwifery in Indonesia?
There are two routes. The vocational route is a D3 midwifery diploma, which produces vocational midwives. The professional route is a midwifery degree (S1 or applied bachelor) followed by the professional midwife programme; Law 17/2023 Article 212 says degree graduates may practise only after that programme. Law 17/2023 Article 210 sets a three-year diploma as the minimum for health workers, so a health vocational school (SMK) graduate must study further first.
Can Indonesian midwives work abroad?
Yes, but the routes are narrow. In the United Kingdom, the midwifery occupation code is eligible for the Health and Care Worker visa, and you must pass the NMC Test of Competence for midwives, show English evidence and be sponsored by an employer. Japan has no midwifery field in its Specified Skilled Worker programme, and its EPA programme is only for nurses and care workers. Treat any midwife job abroad that is not listed on SISKOP2MI with suspicion.
Where do midwives work in Indonesia?
Midwives work in community health centres (puskesmas) and their networks, public and private hospitals, clinics, and independent midwife practices. At village level, Government Regulation 28/2024 Article 512(3) requires every village or urban-ward health service to be staffed by at least one nurse and one midwife, so the village midwife (bidan desa) role remains important. The regulation also allows independent practice, with STR and SIP numbers on display.
How do midwives join the civil service, PPPK or special assignments?
Midwife is a civil-service functional position under PermenPANRB 36/2019, with skilled grades (Terampil to Penyelia) for D3 graduates and expert grades (Ahli Pertama to Ahli Utama) for professional graduates; first appointment requires an STR. CPNS and PPPK posts open through SSCASN. The Ministry of Health's announcement of 15 July 2026 for period I of its special assignments lists midwives among the health workers needed, for a two-year assignment with no recruitment fee.
Is the Midwifery Law still in force after the 2023 Health Law?
Law 4/2019 on Midwifery was revoked by Law 17/2023 on Health (Article 454(k)). Midwives are now regulated in Law 17/2023 alongside other health workers, with details in Government Regulation 28/2024. To avoid a legal gap, Article 453(j) keeps the Midwifery Law's implementing regulations in force as long as they do not conflict with Law 17/2023. In practice, some older technical rules on midwifery practice still apply until they are replaced. STRs and SIPs issued before Law 17/2023 remain valid until they expire.
Who handles the STR, the competency exam and midwifery standards?
Three bodies matter:
- The Indonesian Health Council (Konsil Kesehatan Indonesia, KKI) issues the STR on behalf of the Minister of Health. According to the KKI website, the definitive STR is now valid for life, and from 12 October 2026 registration runs only through SATUSEHAT SDMK. KKI also issues a Certificate of Good Standing, which overseas regulators sometimes ask for.
- The collegium (kolegium) is an independent body of the Council whose members are professors and experts. Under Law 17/2023 Article 272, collegia set competency and curriculum standards. The midwifery collegium also runs a recertification exam for midwives returning to practice, announced through KKI.
- Education providers run the students' competency exam together with the collegium. According to KKI, the 2026 national exam for D3 and professional midwifery students is computer-based, and study programmes handle registration.
Midwifery associations still exist, and you are free to join for networking and professional development. Law 17/2023, however, does not make an association's recommendation a condition for the STR or SIP.
How do you keep your STR and SIP valid?
The STR does not need renewing, but the Council or a court ruling can suspend or revoke it. The SIP lasts five years and is renewed with the STR, a place of practice and enough professional credit units (SKP), so keep a record of every training and development activity. One SIP covers one place of practice, so changing workplace means applying for a new SIP. Never lend your STR or SIP to someone else or to a clinic that does not employ you: Law 17/2023 bars practising without an STR or SIP and bars employing health workers who lack an SIP.
How should you read the civil-service and PPPK pay table for midwives?
The pay table above shows civil-service (PNS) base pay under Government Regulation 5/2024 for two grade ranges: grades II/c to II/d, which match the skilled grades for D3 graduates, and grade III, which matches the expert grades for professional midwifery graduates. Base pay rises with grade and years in grade, and a civil-service midwife's actual income also includes functional, family and regional allowances. Contract civil servants (PPPK) have a separate pay table under Presidential Regulation 11/2024, organised by PPPK grade; because we have not verified the rule that maps education to PPPK grades, we do not show those figures yet. For midwives in private hospitals or clinics, KarirAman has no data that meets our standard, so the minimum wage where you work remains the floor.
What are working conditions and on-call duty like?
Births can happen at any hour, so midwives in hospitals and inpatient health centres usually work rotating shifts, while village midwives and those in independent practice are often on call outside working hours. Law 17/2023 Article 273 gives health workers the right to fair pay, occupational safety and health protection, and health and employment social security. If you work for a private employer, it must register you with BPJS (Law 24/2011 Article 15), and Government Regulation 35/2021 requires overtime pay when hours exceed the rules. Ask for a written employment agreement that states the shift pattern before you start.
What does a midwife's career path look like?
In the civil service, PermenPANRB 36/2019 sets the skilled grades Bidan Terampil, Bidan Mahir and Bidan Penyelia for D3 graduates, and the expert grades from Bidan Ahli Pertama to Bidan Ahli Utama for professional graduates. In the private sector, midwives can move up to coordinating midwife, head of the delivery ward or clinic manager. Other options are opening an independent practice with your own SIP, teaching at a midwifery school after further study, or working in maternal and child health programmes. To move from vocational to professional midwife, you need a midwifery degree followed by the professional programme.
What are realistic options for working abroad?
Midwifery is rarely an official overseas work route for Indonesian workers. The clearest written route is the United Kingdom: the midwifery occupation code is eligible for the Health and Care Worker visa, but you must join the Nursing and Midwifery Council (NMC) register through the Test of Competence for midwives, provide English evidence and be sponsored by an approved employer. Japan does not open midwifery as a Specified Skilled Worker field, and its EPA programme is only for nurse and care worker candidates.
Some midwives choose elderly care work (caregiving) abroad. That is a different job with a different scope, not practice as a midwife; read our caregiver guide before deciding. Whatever the route, Law 18/2017 Article 30(1) says Indonesian migrant workers cannot be charged placement costs, Ministerial Regulation P2MI 17/2025 allows charging the worker only for costs written and itemised in the Placement Agreement, and any request for money outside it is a red flag. Which documents should you prepare when applying?
Prepare copies of your diploma, competency certificate, STR and, if you have them, earlier SIPs and training certificates. Hand over copies, not originals: Ministry of Manpower Circular M/5/HK.04.00/V/2025 bars employers from holding original diplomas and personal documents as a guarantee. The interview is held in a hotel room, a private house or another closed place that is not an office, or the location keeps changing. An invitation to stay overnight, or any sexual demand, as a condition for being hired or for renewing a contract is also a warning sign.

Which job scams target midwives in Indonesia?
The most damaging trick is a promise of a job at a hospital, health centre, in the civil service or on a remote posting in exchange for a processing fee; in 2026 Karanganyar police named suspects in such a hospital job case. Also watch for fast STR or SIP services and paid training that misuses the Ministry of Health name. The ministry says its special-assignment recruitment for health workers charges no fee.

Does a midwife's STR need renewing in Indonesia?
No. Under Law 17/2023 Article 260(4), the STR is valid for life unless it is suspended or revoked. The SIP is what you renew: it lasts five years and needs the STR, a place of practice and enough professional credit units (Article 264). Under Government Regulation 28/2024, one SIP covers one place of practice. According to the KKI website, registration moves to SATUSEHAT SDMK from 12 October 2026.
Is Indonesia's Midwifery Law still in force?
No. Law 17/2023 Article 454(k) revoked Law 4/2019 on Midwifery. Article 453(j), however, keeps its implementing regulations in force as long as they do not conflict with Law 17/2023. So the main legal basis for midwives is now Law 17/2023 and Government Regulation 28/2024, plus older rules not yet replaced.
Must midwives join a professional association to get an STR or SIP?
Law 17/2023 does not make association membership a condition for the STR or SIP. Article 260 lists the STR requirements as a diploma or professional certificate plus a competency certificate, and Article 264 lists the SIP requirements as the STR and a place of practice. Article 311 only says health workers may form professional associations. Joining remains your choice.
Can a D3 midwifery graduate start work straight away?
Yes, once you pass the competency exam and hold an STR. Under Law 17/2023, vocational graduates who pass the national competency exam receive a competency certificate, which the STR requires. According to KKI, the 2026 national midwifery exam is computer-based and registration goes through your study programme, not individually. To practise at a given place you also need an SIP.
Can anyone ask me for money to get a midwife job?
No. Indonesia's Ministry of Manpower says a recruitment fee is almost certainly a scam, and the Ministry of Health says its special-assignment recruitment for health workers is free. In 2026 Karanganyar police named suspects who promised hospital jobs in exchange for a processing fee. BKN says civil service selection is free too. Refuse and report it.
How can I check that a midwife's practice has an STR and SIP?
Government Regulation 28/2024 requires health workers in independent practice to display their name, STR number, SIP number and practice hours, and health facilities must publish a list of their health workers with STR and SIP numbers. If the numbers are not shown, ask. Law 17/2023 Article 312 bars anyone from giving the impression of holding an STR or SIP without the right to do so.
